I understand the studio's role. The artists at Da Vinci Tattoo are professional tattooers, not medical professionals. They cannot provide medical advice, evaluate health risks, or handle medical complications.

I am clear from the following conditions (or have doctor clearance to get tattooed):

  • Bleeding or clotting issues, or taking blood thinners/aspirin
  • Heart conditions, blood pressure issues, or history of fainting/seizures
  • Diabetes or conditions that cause slow healing
  • Active skin issues in the tattoo area (eczema, psoriasis, rashes, severe scars)
  • Allergies to medical bandages (Saniderm/Second Skin), numbing creams, latex, or pigments
  • Bloodborne infections (such as Hepatitis or HIV)
